MADISON COUNTY SHERIFF’S OFFICE – REPORT REQUEST INSTRUCTIONS:

Note: The Madison County Sheriff's Office will process requests for Ennis Police Department historical reports and calls for service. If you know your request is for an Ennis PD incident, please check the appropriate box on the form.  

  1. A request to the Sheriff’s Office for an accident or police report must be in writing - dated and signed.
  2. The written request should provide as much information as possible to locate the particular report you need:  date, location, names of parties involved, deputy’s name (if known).
  3. There is a $2.00 fee for each report being requested – this fee is to be paid to the Madison County Sheriff’s Office in advance (additional cost for statements, photos, etc.).
